Intake Specialist CCR

The Intake Specialist – CCR works directly with participants and their families in applying to receive services through the CCR an ACCESS program with short-term and long-term solutions. This position works collaboratively with a team that assists participants in navigating through complex resources and processes to obtain/sustain housing. An Intake Specialist works to assist the participants through the application process to   determine program eligibility and case manager placement. Housing Stabilization Case Manager, CCR

The Housing Stabilization Case Manager in CCR (Center for Community Resilience) works as part of a multi-disciplinary team of community partners and other ACCESS staff working together to support participants and families who have been impacted by the fires to gain or maintain housing stability This will include, but not be limited to, populations such as seniors, disabled, and our most vulnerable neighbors who are unhoused or at-risk of becoming unhoused. Veteran’s Services Housing Stabilization Case Manager

Position Summary
The Veteran’s Services Housing Stabilization Case Manager (SSVF) works directly with Veterans and their families to overcome barriers that may prevent them from obtaining or maintaining housing stability. The Veteran’s Services Housing Stabilization Case Manager works collaboratively with a multi-disciplinary team that assists Veterans and their families to navigate complex resources and processes that can help them achieve their own definition of self-sufficiency. This position works in the office environment as well as out in the community and in participants homes to offer wrap-around services. This position reports directly to the Veteran’s Services Manager.

Position Summary
The Intake Specialist- Support Services for Veteran Families (SSVF) works directly with Veterans and their families in applying to receive services through ACCESS with short-term and long-term solutions. The Landlord Engagement Specialist in Supportive Services for Veteran Families works as part of a multi-disciplinary team of outreach, intake, case management, and program support working together to address Veterans’ housing stability needs in Jackson County This position will locate and secure permanent housing opportunities, for program participants, as quickly as possible through outreach to landlords, property managers and housing authorities. This position is responsible for providing positive and professional service to all ACCESS participants.
